Parents are encouraged to check this page to ensure that they are submitting all forms for their child's camps. Regardless of registration method (online or in-person), Camp Forms Packet must be completed and submitted no later than May 15. All forms are listed below. Camp Registration Form – Required for in-person registration, including for all Fee Waiver Applicants.
Camp Forms Packet – Must be completed and submitted for all campers - regardless of camp - no later than May 15. Failure to do so will result in forfeiture of your child's space. Participants only need to submit one set of forms, regardless of the number of camps in which they are enrolled.
Medication Authorization Form – This form must be completed for prescription and non-prescription medications. Medication, including over the counter medication, will be given to a child only with a parent’s or guardian’s completed written consent.
